(MS)]
[Briefly describe the project specific scope
of the Work covered herein the MS]
[Briefly describe the personnel and/or
Parties that shall be responsible for usage of this MS. Communication plan
shall be identified herein for which the Parties shall be responsible properly
execution of issuance, approval and/or updating of this MS]
[Provide explanation of any work specific clause,
wording and abbreviations that is mentioned herein the MS]
[Provide quantity and qualification ( incl. trainings received
prior to work) of indirect and direct personnel.Respective Planned Manpower Distribution Histogram
shall be provided as an attachment.]
[Qualification shall be especially provided for the Key Personnel (such
as Site Manager, QA/QC Manager, HSE Manager, work specific Site Superintendent,
Engineers, Supervisors and skilled labors,etc.).Provide work specific
certificates for the employed indirect and direct personnel.]
[Provide list of project
materials, tools and consumables that shall be supplied by the Subcontractor
for the execution of the Work defined herein the MS]
[Provide list of project
materials, tools and consumables that shall be supplied by GAMA for the

construction equipment as an attachment that shall be supplied by
Subcontractor for the execution of the Work defined herein the MS. Equipment
properties such as but not limited to type, model, capacity, certification
(equipment,,operators tools,etc.), etc. shall be identified. ]

[Provide list of equipment
as an attachment that shall be supplied by GAMA for the execution of the

for personnel, materials, equipment to be supplied by sub-tier subcontractors as

approval as per the requirements agreed and signed by the Subcontract

for each of the subject activities including responsible personnel or sub-tier
contractors’ duties and responsibilities. Describe the entire process in

preparation activities, such as mobilization, demobilization and access to Work

[Express sequence of all
activities to be used for proper execution and completion of the Work]
[Describe the methods and
techniques for the construction activities such as, site preperation, dewatering,
excavation, backfilling,piling,underground piping and utilities, formworks,
reinforcing, scaffolding, earthing, cathodic protection, concreting, grouting, heavy
haull, inland transportation, preservation and storing, offshore works, steel structure
erection, cladding, architectural finishing works, roads, paving and
landscaping works, mechanical erection, equipment installation, levelling, piping
prefabrication and erection, support works, painting and insulation, HVAC and Fire Fighting system (incl.detection,
protection) erection, electrical installation, cable laying,cable pulling,
cable termination, lighting and lightning protection, communication system
erection, instrumentation, pre-commissioning, commissioning, etc., that are to
be valid and covered as per the scope of the Work defined herein the MS.]
[Explain in detail each
unique activities including the methodology to be used for proper execution and
completion of the Work]
[Explain in detail the
allocation of above foreseen resources in articles 5.,6 and 7 for each of the respective
activities of the Work.]
[Provide illustrative
skecthes, technical drawings, calculations, etc., as an technical appendix
for proper execution and completion of the Work]
[Determine the work
breakdown structure, activity durations, start and finish dates for each of the
respective activities of the Work.]
[Provide detailed document
such as marked up diagrams, drawings, skecthes, excel lists, etc., to describe
monitoring of each of the Work activities on daily basis.]
[Explain in detail progress
measurement method that shall be recommended by the Subcontractor for GAMA’s
approval if it is not defined in the agreed and signed Subcontract Agreement.]
[The Subcontractor shall strictly comply
with the Project QA/QC Plan.]
[Describe the implementation of the QA/QC Plan consisting pre-inspection,
monitoring, final inspection stages. Provide associated documents
such as Quality Control Plan, I&T Plans, QA/QC procedures, turn over packages,
internal quality audit schedule etc., for proper execution of Work]
[Provide the name, contact information, company details, qualifications,
references and scope of the Subcontractor’s sub-tier contractors such NDT, 3rd
Party Inspection, etc., and responsibilities.]
[Provide information (CVs, certificates, contact information, etc.) for
the assigned QA/QC team who shall be responsible for carrying out the quality
activities in coordination with GAMA Quality Department.]
[Briefly describe issuing, recording, reporting and corrections of non-conformities during the execution of Work]
[Provide the list of control and test equipment and tools. Identify
type, model and calibration certificates as an attachment that shall be
supplied by Subcontractor for quality control of the Work.]
[GAMA and subcontractors’ safety documents such as Health, Safety and
Environmental (HSE) Plans including procedures, inspections, audits that will
be followed during all project related activities will be referred/explained
herein.]
[Work specific mitigation measures with regard to HSE requirements shall
be identified herein]
[Provide list of work specific personnel protective equipment which
shall be used during performing the work, such as special type breathing
devices, etc. Personal Protective Equipment (PPE) properties such as but not
limited to type, model, certification, etc. shall be identified.]
[Work specific environmental requirements and the necessary
equipment/tools being used shall be identified including the waste management
measures/procedures that will be followed.]
[Work specific topics for toolbox, weekly, monthly meetings will be
identified and explained herein order to help the immediate supervisors conduct
to the people that will be working for the work.]
[If the work requires any lifting operation, lifting checklist attached
hereto shall be used]
[Provide list of work specific permits necessary to perform the work herein]
[A work specific risk assessment in accordance with the Projects Risk
Assessment Procedure shall be prepared which will be covering all steps of the work
and reducing all the risks to an acceptable level. The prepared Work Risk
Assessment documents shall be attached to the MS.]
[Lifting Method and Risk Assessment shall be prepared in accordance with
the sample provided in Attachment-7, if applicable for the Work, and shall be
attached to the MS.]
[List the document code, number and title referred herein MS required to
execute the Work]
[List the technical documents produced by the Subcontractor and/or
Vendor and provide the documents herein MS]
[List the aforementioned attachments and provide the documents herein
MS]
